All insurance policies differ; therefore, we cannot give you a definitive answer to this question without verifying your individual insurance policy. After your initial evaluation, we will verify insurance coverage and benefits. Patients are encouraged to check with their insurance company prior to their appointment regarding their benefits for our services.
Each device costs are different. Once a patient is evaluated by a practitioner and a specific type of device and design are determined to be appropriate, a total price can be determined. We provide the patient with a price quote at the end of an evaluation appointment with anticipated total price of the device.
A patient is responsible for any adjustment, modification, or repair charges after ninety (90) days from the date which the orthotic/prosthetic device is delivered unless warranty agreement specifies otherwise.
Excell Orthotics & Prosthetics is licensed by the State of Texas, and as a licensed facility we are required to have a physician's prescription for any evaluation or device that we provide. In addition, all insurance companies require prescriptions and Letters of Medical Necessity to establish necessity for payment.
We offer an initial free evaluation appointment with a prescription from your doctor.
We are "in network" with Medicare and Blue Cross Blue Shield. For insurances that are "out of network", we will be glad to work with you to file with your insurance for reimbursement available. We also suggest that you contact your insurance provider to check if you have "out of network" benefits if we are not "in network" with your insurance company.
